THE MEDICAL JOURNAL OF AUSTRALIA 929 obstructive jaundice in the absence of extrahepatic obstruction, as demonstrated at laparotomy. A liver biopsy taken at the time of surgery revealed bile stasis and focal nodular histiocytic lesions resembling eosinophilic granulomas within inflammatory scar tissue surrounding the biliary radicles. Necropsy material four weeks later revealed only a few remalning involuting histiocytic nodules in portal areas, but a striking increase in the degree of portal fibrosis and bile stasis. The authors state that disseminated histiocytosis X should be included in the ditIerential diagnosis of obstructive jaundice, with the realization that the pathogenesis may be one of intrahepatic biliary obstruction, due to histiocystic lesions accompanied by rapidly developing portal fibrosis.
