State of Michigan Form W4P. In 2011 the State of MI passed a law requiring all annuity or IRA distributions to have state tax withheld, regardless if you actually owed the tax. If you were born before 1946 you do not own the tax and should fill out this form & send it in to your investment firm so they they do not withhold the state tax. I am not a tax advisor.
