What is All That 'plops’ Is Not Always A Mass about?

Background: In large atrial masses, phonocardiographic timing of a tumor ʻplopʼ has been correlated with motion of the cardiac mass. The sound is produced by the strong impact of the mass on the interventricular septum (IVS) in diastole. This case illustrates how an elongated, redundant anterior mitral valve leaflet (AMVL) can mimic the appearance of a valve mass and produce a sound similar to a tumor plop. Case: A 39-year-old male presented with a 2-month history of a progressively enlarging scrotal mass and unintentional weight loss. Workup revealed a seminoma with extensive local, thoracic, and intracranial metastases. Neoadjuvant chemotherapy and radiation were initiated. Auscultation of a tumor ʻplopʼ prompted a TTE which revealed an independently mobile echo density on the atrial side of the AMVL (Fig A) with mild mitral regurgitation raising suspicion for intracardiac metastasis. Decision-making: TEE (higher resolution than TTE) was done for further evaluation and excluded a discrete valve mass. Rather, it showed an elongated, redundant AMVL (Fig B) with prolonged early diastolic IVS contact (Fig C), timed with the diastolic sound on phonocardiography (Fig D). Conclusion: Th
