Background: CRT is part of the standard treatment for locally-advanced rectal cancer (LARC), but its effects on patient-reported QOL are not well described. As radiation oncologists, we have the opportunity to report such effects, and explore potentially modifiable factors (such as dose to normal structures) associated with adverse outcomes. Materials/Methods: We prospectively collected patient-reported symptoms in 56 relatively homogeneous patients with LARC receiving radiation with concurrent 5-FU chemotherapy. Patients typically received 50.4 Gy via 3D planning, and were treated in the prone position. Immediately before the weekly physician encounter, patients completed a previously validated (prostate cancer) 7-item bowel symptoms survey (diarrhea, urgency, rectal pain/irritation, rectal bleeding, cramping, mucus discharge, tenesmus), with each item based on a 5-point Likert scale (from ''not at all'' to ''very frequently''). Using the RTOG Acute Morbidity Scoring Criteria, physicians graded diarrhea weekly. Results: Median age was 49. Forty-two patients received neoadjuvant CRT, and the other 14 adjuvant CRT.
