Umbraco Contact Us Module Guide by Nelson is a document available to read on EtoBox.

This Umbraco module allows users to submit contact information from anywhere on a site. It can send the information via email by configuring SMTP settings, or store it under a content node. The module uses jQuery and jQuery validation libraries, and comes with a default Contact Us template and document type that can be customized. It is currently in alpha version 1.1.
