Background: Visceral leishmaniasis (VL), or kala-azar, is a life-threatening neglected tropical disease caused by Leishmania donovani and transmitted by infected sandflies. Despite significant advancements in public health, the disease remains a major global health challenge, predominantly affecting populations in South Asia, East Africa, and South America.Methods: This study conducted a systematic review following PRISMA guidelines to analyze the current epidemiology, diagnostic advancements, therapeutic innovations, and vector control strategies for VL. Data were extracted from peer-reviewed articles published between 2000 and 2024, retrieved from databases such as PubMed, Scopus, and Web of ScienceResults: The review identified substantial progress in diagnostics, including the widespread use of the rK39rapid diagnostic test and advancements in molecular diagnostics. Treatment has improved with liposomal amphotericin B and miltefosine, though drug resistance and accessibility remain concerns. Vector control strategies, such as indoor residual spraying and community-based interventions, have proven effective but face challenges due to insecticide resistance. Co-infections with HI
