1) This case report describes a 23-year-old man who presented with back and abdominal pain as the initial and sole manifestation of systemic lupus erythematosus (SLE). 2) Neurological examination revealed signs of myelopathy and MRI of the spine showed short segment hyperintensities in the cervical and thoracic spinal cord. 3) Laboratory tests confirmed the presence of anti-Smith antibodies and a diagnosis of SLE-associated myelitis was made, with the myelitis as the first sign of SLE.
