What is Sunitinib: Acute interstitial nephritis: case report about?

## Acute interstitial nephritis: case report A 69-year-old man developed acute interstitial nephritis (AIN) following treatment with sunitinib [route and time to reaction onset not stated] for metastatic renal cell carcinoma. The man, who had a history of stage IV left kidney clear cell carcinoma with metastases to spine and brain, presented with a 4 day history of gross haematuria, oliguria, fever (38.5°C), fatigue and a decreased appetite. He had been started on treatment with sunitinib 50mg daily at bedtime, 2 weeks ago. Despite withdrawal of aspirin [acetylsalicylic acid] and dipyridamole, which were prescribed for a previous stroke, his haematuria persisted. His sunitinib therapy was discontinued and a course of amoxicillin for presumed urinary tract infection, was initiated. Upon admission, his BP was found to be 160/72mm Hg. Laboratory investigations revealed mild anaemia (haemoglobin of 11.9 g/dL), thrombocytopenia, hyponatraemia (sodium of 120 mEq/L) and acute renal failure (BUN of 41 and creatinine of 2.7 mg/dL). A urine dipstick was positive for blood and protein. He showed low urine sodium (10 mEq/L) consistent with dehydration. Total urine protein collected in 24 hours
