What is Photon IMRT in Adenoid Cystic Carcinoma with or without Carbon Ion Boost: Results in 116 Patients from a Single Institution about?

Materials/Methods: After obtaining IRB approval, we identified 89 total laryngectomy specimens, which had preoperative contrast enhanced CT scans of the neck reviewed at our institution. Specimens previously irradiated, and those of subglottic or pyriform sinus origin were omitted from review. Final pathology and radiology reports were coded according to observed extent of disease. Pathological findings were considered the gold standard with radiological data providing supplemental information if findings were not appreciated ex vivo. Routes of spread were categorized as anterior (TC penetration), posterior (arytenoid cartilage destruction and thyroarytenoid space widening), inferior (conus elasticus penetration) and superior (base of tongue (BOT) involvement, thyrohyoid membrane (THM) and thyroid notch penetration). Results: Eighty-nine TL cases were reviewed and 52 cases of ELS were identified. Patterns of invasion were as follows: Anterior Spread through the TC-25%, Posterior spread-20%, Inferior spread-15% and Superior spread-15%. Conclusions: While TC penetration is a common route of ELS of glottic and supraglottic SCC, ELS into surrounding structures without TC penetration oc
