What is Multiple drugs: Lack of efficacy and drug-related fast waves: 2 case reports about?
Multiple drugs Lack of efficacy and drug-related fast waves: 2 case reports In a cohort study, 2 patients, a 4-year-old boy and a 14-month-old infant were described who exhibited lack of efficacy during treatment with diazepam, valproate, phenobarbital, propofol or midazolam. Of whom, a 14-month-old infant exhibited drug-related fast waves during treatment with midazolam.Case 1: The 4-year-old boy developed febrile seizures at the frequency of 1-2 times yearly, since age of 1 year. At age of 4 years, he experienced febrile induced status epilepticus, which lasted for 1h and stopped after the administration of IV infusion of valproate 1.5 mg/kg/h. Despite disappearance of the seizures, he was in a state of coma. His Glasgow Coma Scale score was 6/15 (E2 + V2 + M2). After 2 days of admission, he developed refractory status epilepticus, as his seizures were unresponsive (lack of efficacy) to IV valproate, diazepam and phenobarbital. He was diagnosed with super-refractory status epilepticus and remained uncontrolled 24h after initiating continuous use of propofol and midazolam (lack of efficacy). Obvious intracranial hypertension of 310mmH 2O was observed. EEG showed diffuse slow waves
