This case series describes 3 severely immunocompromised patients from whom influenza virus variants resistant to antiviral drugs were recovered. Two patients were infected with influenza A virus that had mutations conferring resistance to oseltamivir and M2 inhibitors. One patient was infected with an influenza B virus with a novel mutation that decreased susceptibility to oseltamivir and zanamivir. Prolonged viral shedding in immunocompromised patients can promote the emergence of antiviral resistance.
