What is Editorial for “Diagnosis of Sarcopenia Using the L3 Skeletal Muscle Index Estimated from the L1 Skeletal Muscle Index on MR Images in Patients With Cirrhosis” about?

S arcopenia is usually defined as loss of muscle mass and muscle function. Sarcopenia has important health and economic implications; it is related to functional disabilities, risk of falling, and a higher rate of outpatient visits in the elderly. 2 Furthermore, it is a common complication of cirrhosis. Sarcopenia is widely diagnosed by calculating the skeletal muscle index (SMI) at the third lumbar vertebral level (L3). This is defined as the skeletal muscle area (SMA) divided by the square of height. The main advantages of MRI for SMI calculation are good soft tissue contrast and high resolution. Assessment of liver parenchyma and sarcopenia in MRI can help to guide treatment and to improve prognosis of cirrhosis. Routine liver MRI protocols are designed to cover the entire liver, and the field of view (FOV) covers just below the first lumber vertebra (L1). Hence, the FOV does not typically include axial slices of L3, posing a limitation in sarcopenia assessment in routine liver MRI scans. However, previous research has shown close correlation of SMA at the 12th thoracic vertebra (T12) with the SMA values at L1 and L3 in a healthy population. In the current issue of JMRI, Xu et a
