This document proposes a fuzzy ontology modeling approach for case base knowledge in diabetes mellitus diagnosis. It presents a novel case-base fuzzy ontology called CBRDiabOnto that contains fuzzy classes, properties and datatypes. The ontology is populated with 60 cases and evaluated for accuracy and consistency in representing diabetes diagnosis terminology and logic.
