This case report describes a rare case of carcinoma arising from the axillary tail of Spence (CATS) in a 46-year-old woman. She initially presented with a painful mass in her right axilla and underwent chemotherapy, but the mass continued to grow. MRI and PET-CT scans revealed a soft tissue mass arising from the right axillary tail with enlarged lymph nodes. The mass was excised along with axillary lymph node dissection.
