The patient, a 70-year-old woman, presented with jaundice and pain in the right hypochondrium. Clinical examination and laboratory tests revealed elevated bilirubin, GGT, alkaline phosphatase, amylase and lipase, consistent with obstructive jaundice and acute pancreatitis. Imaging including ultrasound, CT scan and MRI cholangiopancreatography showed dilation of the common bile duct and pancreatic duct, suggesting a neoplasm at the head of the pancreas compressing the ampulla of Vater.
