This case report describes a 49-year-old woman who developed orbital apex syndrome as a rare complication of herpes zoster ophthalmicus infection. She initially presented with symptoms of lacrimation, foreign body sensation, and eyelid edema in her left eye. Despite treatment with oral brivudine and topical acyclovir, her symptoms worsened with loss of vision and complete ophthalmoplegia developing within 3 days.
