Marcelo M. Oblan III purchased a laptop from Octagon Computer Store on January 6, 2023 and was issued a receipt. He later disposed of the receipt inadvertently thinking it was rubbish. Despite searching diligently, he was unable to find the lost receipt. As such, he executed an Affidavit of Loss on January 24, 2023 to attest to losing the receipt and to request a new one be issued in its place.
