A study was conducted at a tertiary care hospital to assess the role of clinical pharmacists in detecting and reporting adverse drug reactions (ADRs). Over a 6 month period, pharmacists identified 50 ADRs. Most ADRs were type A reactions and involved antibiotics. High drug dose/frequency, age, and polypharmacy were common risk factors.
