Background: Papillary muscle rupture is a lethal mechanical complication (MC) of acute myocardial infarction (AMI). Rupture of the anterolateral papillary muscle (APM) is rare, and usually follows an ischemic event involving the circumflex or left anterior descending arteries. Rupture as a result of an isolated right coronary lesion is anecdotal. Case: A 72-year-old male with history of hypertension was admitted to our center on account of an inferior ST-segment elevation AMI. Coronary angiography revealed diffuse ectasia and total occlusion of the posterior descending artery. Due to high thrombus burden, the percutaneous coronary intervention was unsuccessful. Five days after admission, the patient developed sudden acute pulmonary edema and hemodynamic instability. A transesophageal echocardiogram demonstrated acute severe mitral regurgitation (MR) secondary to APM rupture. ## Decision-making: In the setting of cardiogenic shock secondary to AMI with MC, an intra-aortic balloon pump was placed as bridge to surgery. Surgical intervention confirmed the APM rupture, and replacement of the mitral valve with a 31 mm bioprosthesis was performed. ## Conclusion: Acute hemodynamic decompen
