This case report describes a 19-year-old male who presented with abdominal pain in the epigastric and right lower quadrant regions. Imaging and labs did not reveal a clear cause. Diagnostic laparoscopy identified lesser omental infarction (torsion of fat tissue in the abdomen) and acute appendicitis. The patient underwent laparoscopic surgery to remove the infarcted omentum and appendix, and recovered well postoperatively.
