This study assessed the effects of advanced periodontal disease on dental pulp structure by comparing 52 teeth with severe periodontitis to 52 healthy control teeth. Teeth with periodontitis showed higher rates of inflammation, fibrosis, calcification, and necrosis in the pulp compared to controls. The severity of pulp changes increased with deeper periodontal pockets. Complete pulp necrosis only occurred when periodontal pockets reached the root
