The piston effect occurs when differential pressure across a downhole packer generates a force. When the packer is set, tubing and annulus pressures are equal and no net force acts on the packer. However, if either pressure subsequently changes, a differential pressure is created, generating a force. This force is calculated based on the initial and final pressure conditions in the tubing and annulus.
