## Background: We report a case of exercise-induced right and left ventricular outflow tract ventricular tachycardia (VT) in a patient with triple vessel coronary artery disease (CAD). Case: A 57-year-old male was admitted after an episode of non-sustained VT during an outpatient exercise stress echocardiogram. An electrocardiogram showed normal sinus rhythm (NSR) with premature ventricular complexes (Figure A). A coronary angiogram showed triple vessel CAD and a stent was placed in the ramus intermedius. The left circumflex had subtotal occlusion and moderate lesions were noted on the left anterior descending, 1st diagonal, and right coronary arteries. An Instantaneous wave-free ratio for the left anterior descending artery was negative. The next day, he had 2 episodes of hemodynamically stable sustained monomorphic VT requiring diltiazem infusion and then later another episode of hemodynamically unstable sustained VT requiring shock (Figure C). Decision-making: An electrophysiological study showed inducible sustained, monomorphic VT which degenerated into ventricular fibrillation.
