Patients with medically refractory gastroesophageal reflux disease (GERD) should undergo endoscopy and esophagram with possible manometry, 24-h impedance, or BRAVO to determine if surgical endoscopic procedure could improve patient's symptoms. Surgical fundoplication has better long-term outcomes compared with endoscopic procedures for the treatment of medically refractory GERD with or without hiatal hernia. Endoscopic procedure (radiofrequency ablation) and transoral incisionless fundoplication provide improved outcomes compared with medical therapy for patients with medically refractory GERD with no hiatal hernia of hiatal hernia <3 cm.
